If You Need Additional Coverage
If you need additional insurance coverage above the amounts of guarantee issue or you want to purchase insurance or disability coverage after the enrollment period of 30 days from the date of hire, you must complete an Evidence of Insurability Form - available by calling 4A's Benefits.
This form should be completed and submitted to 4A's Benefits via fax or email.
We will review the form to check for any missing information and forward all the information to the insurance carrier for the underwriting coverage decision.
Please note that the insurance carrier may require you to provide additional information at the your expense.
